The Southern Delaware COPD Coalition was founded by a registered respiratory therapist who, after working in local hospitals for nearly 20 years and witnessing firsthand the revolving door of hospital readmissions for people with chronic lung disease, decided something different was needed to improve the quality of life for people with chronic lung disease. While some patients were fortunate enough to have the necessary support and resources to properly manage their health, this was not the norm. Many patient stories share a common theme: resources are limited in the outpatient (or home) setting and many people are unable to access the services/medications they need to effectively manage their lung disease. These factors ultimately increase the risk of readmission to the hospital and lead to decreased quality of life and poorer outcomes.
The Southern Delaware COPD Coalition's mission is to eliminate disparities and increase access to healthcare, to improve the quality of life for people with COPD and other chronic lung diseases. We recognize the importance of a holistic approach to effectively manage the complex healthcare needs of this population. Our mobile pulmonary navigator assesses each patient's needs, identifies barriers to care, and works with our community partners to ensure any needs or barriers are addressed. We work to bridge gaps in care and ensure patients with chronic lung disease have access to the healthcare services they need including testing, education, screening, appointments, medications, treatment, and therapies.
To continue our efforts and fulfill our mission, your support is needed.
